Our client is a not for profit organisation with a vision of everyone being part of a connected care community. They assist clients in being assessed for and receiving the best possible care whilst managing and delivering on a range of government or other funded programs.

The Role
  
Reporting to the CEO, you will be responsible for managing a small non clinical team and undertake the following duties:

  • Oversee the homecare assessment processes
  • Support the hospital care team with health care plans
  • Maintain and nurture external stakeholder relationships
  • Manage programs operational aspects, to ensure funding agreement deliverables are met
  • Provide an efficient quality service to clients and meet related contractual obligations

Skills Required
  
You will be an experienced people manager with an understanding of the aged care sector and have:

  • Knowledge of referral pathways, reablement principles, case management, assessment services and service coordination
  • Familiar with non-clinical contract management
  • Leadership experience delivering high quality health plans
  • Ability to analyse funding contracts, agreements and deeds
  • Valid Police Checks and Working with Children Check
