SayPro Refugee Support Case Management

SayPro work with clients to develop a strengths-based, goal-orientated service plan to support and empower refugees on their path to community integration and self-sufficiency.**· Provide comprehensive case management applicable to individualised service plans to advocate and coordinate access to resources; connect clients to medical and mental health care; apply for job placement opportunities; conduct home visits.· Provide all services in a linguistically and culturally appropriate manner for each client utilising interpretation/translation services as needed.· Maintain up-to-date, working knowledge of best practices in trauma-informed, client-centred case management.· Maintain an up-to-date and nuanced understanding of program contracts and requirements.· Collaborate across agency programs and cultivate relationships with other community organisations to ensure client and agency goals are met. Provide referrals to clients as needed.· Ensure accurate, timely and detailed compliance with data entry, client records and program reporting.
