  • SayPro Intensive Case Management Caseworker

     Job Overview: The Intensive Case Management (ICM) Health Caseworker will address barriers to health and wellness for especially vulnerable clients. The ICM Health Caseworker will be expected to provide strengths-based, client-centered case management, regarding client’s identified health concerns, to single-headed refugee households; survivors of domestic violence and gender-based violence; older refugees; youth and young adults without parents or permanent guardians; refugees experiencing mental health or psychological difficulties; refugees with physical disabilities or medical conditions; unanticipated refugee arrivals and secondary migrants; and refugees who identify as lesbian, gay, bisexual, transgender or queer (LGBTQ). They work closely with clients, colleagues, and external parties to develop service plans to meet individual client healthcare needs and improve access to healthcare and resources in accordance with client eligibility and program requirements. Major Responsibilities: Responsibilities include, but are not limited to: ·Collaborate closely with the ICM team to identify healthcare needs and coordinate services for vulnerable clients. ·Assist clients in navigating healthcare systems by supporting health plan and service provider selection. Assist clients in selecting healthcare providers and help schedule initial appointments. Ensure each client is comfortable with contacting their specialist and with scheduling their own appointments moving forward. ·Conduct regular outreach to healthcare providers and social service agencies to address health needs and identify resources. Advocate on behalf of clients who are having difficulty accessing services. ·Develop and distribute healthcare guides tailored to each client. Provide clients with extended orientation on a variety of topics regarding health such as women’s health, specialized treatment for people with physical disabilities, and navigating physical healthcare systems at the local level. ·Respond to client health-related crises, with guidance from supervisor as needed. ·Maintain up-to-date documentation in client’s case file to ensure services are properly documented throughout the client’s service period; document all services provided in IRC’s database, Efforts to Outcomes (ETO). ·Attend relevant trainings and meetings at the local and national level. ·May train and lead the activities of volunteers and interns. ·Other duties as assigned. Job Requirements: Education: Bachelor’s degree in social work, public health, or related field of study preferred. Work Experience: ·Minimum of 1-2 years of case management experience with similar populations and/or client needs required.   • SayPro Casework Supervisor

     Job Overview: The Casework Supervisor is responsible for supervision of staffing and oversight of direct service programs. This oversees quality control for both service provision and contractual compliance. This includes overseeing new arrivals and the provision of core services within required timeframes, maintaining program compliance, coordinating case file reviews, completing program reports, and overseeing the distribution and reconciliation of client funds. The Casework Supervisor also participates, as required, in program planning and staff development through coordinating local training, onboarding new staff, conducting annual performance evaluations, and building and maintaining community partnerships. Major Responsibilities: Responsibilities include, but are not limited to: Program Oversight Supervise program staff to ensure that all core services are provided within the required timeframes, including Cultural Orientation, health screenings, enrollment in benefits, enrollment in schooling, family budgets and referral to job placement programming and English as an Other Language service; Ensure clients are placed in safe, appropriate, permanent housing as efficiently as possible. Ensure housing procedures are followed per PRM and IRC guidance; Regularly visit IRC housing sites for quality assurance; Ensure use of external funds are applied as much as possible to offset housing costs. Track use of funds; Conduct regular and timely case file reviews with team members; Ensure compliance with case file documentation and reporting requirements; Provide training and dissemination of program updates of donor requirements through on-going review of the Cooperative Agreement and organizational announcements; Oversee allocations and complete assurances; Oversee Minor cases, including documentation, home visits, and SRC letters; Assist with providing data and reports for local leadership and IRC HQ, including annual data match, housing audits, and other reports, as requested; Meet with clients regularly to ensure positive experience and to regularly receive client voice and feedback; Support client grievance when appropriate; Oversee program spending, flex funds, and required funds distribution. Reconcile spending for all arrivals; Supervision Meet regularly (once or more times a week) with staff to assign duties and ensure that tasks are completed; Coordinate team direct service meetings; ensure communication and collaboration across departments; Supervise direct service staff, Interpreters and Casework Assistants, including recruitment, onboarding, and oversight which includes onboarding, training and support of new hires, performance evaluations, and related HR tasks; Ensure all staff have updated development plans and work to ensure appropriate diversity, equity and inclusion measures are implemented; Approve timesheets and overtime. Community Engagement Work with team members to utilize referral systems for participants to access outside services; Work with volunteer coordinator to establish volunteer opportunities related to case management; Ensure team is effectively using and managing interns and volunteers; Represent in internal and external meetings, as necessary. Direct Service Conduct pre-arrival services (locating and securing suitable housing, purchasing housing essentials, furniture and basic food supplies, setting up utilities etc.); Provide individualized and group orientations and supporting clients through social service referrals, medical access and advocacy services by assessing and evaluating each client’s distinct needs; Independently develop reasonable resettlement and/or self-sufficiency plans and timelines for each client to facilitate client expectations; Support clients with direct assistance distribution and tracking; Ensure that clients are provided the basic tools vital to meet their established short-term goals and objectives; Assess and monitor client progress at regular intervals to ensure progress on goals is made; designated resources are maximized; modifying initial resettlement plans and other case file documents as required; Complete all necessary data entry and filing in hard-copy case files and databases. Job Requirements: Undergraduate degree with an emphasis in Social Work or an equivalent field of study; equivalent experience may be substituted; Three to four years of relevant work experience. A significant portion of this experience should include responsibilities in social services, humanitarian relief, public service, or other not-for-profit environment; 2 years of supervisory experience preferred; A proven record of increasingly responsible roles in a mission and outcome driven environment; Highly organized self-starter with proven success handling multiple priorities, making critical decisions and delivering results in a fast-paced environment with tight deadlines; Demonstrated success achieving goals and communicating effectively a multi-cultural environment; Strong communications skills, both oral and written, are required for this role; Fluent in English, both spoken and written; other language ability in one of the predominant languages of the local client base preferred; Proficient in Microsoft Office applications (Word, Excel, Outlook); Commitment to DEI principles and an inclusive work environment; Valid driver’s license, reliable vehicle with current insurance, and the ability to travel regularly throughout the service delivery area. The Health and Wellness Caseworker will report to the Casework Supervisor, collaborate with the other members of the Casework team and, as needed, supervise volunteers and interns. Major Responsibilities: · Schedule two required medical screenings for all newly arriving refugees within first 30 days of arrival. · Schedule first primary care appointment for all new refugees to establish care. · Assist clients in completing required immunizations within 1st year of arrival. · Schedule follow up medical services appointments as needed. · Help clients coordinate transportation to / from appointments. · Track and report out on client attendance to appointments. Work to understand and overcome barriers to attending appointments. · Complete SSI applications for elderly and permanently disabled refugees. · Maintain separate confidential client medical files according to established standards. · Timely completion of client records, documentation and case files according to IRC policies. · Oversight of volunteers and interns in completion of delegated tasks. · Track specialist referrals and assist in scheduling appointments. · Manage and schedule interpreters for specialist appointments. · Develop and maintain productive relationships with medical providers, community partners, and other social service agencies and advocate for the opportunity for refugees to have full, ongoing access to medical, mental health and mainstream providers. · Ensure compliance with case file management and reporting requirements. · Other duties as required.
